Intake Specialist

Pay Rate: $18 Per Hour
Summary
The Intake Specialist is the first point of contact for clients seeking services. Responsible for screening referrals, collecting and verifying client information, assessing initial eligibility, and connecting clients to appropriate programs and resources while maintaining confidentiality and accurate records.
Responsibilities
· Conduct comprehensive biopsychosocial assessments and develop individualized, person-centered treatment plans.
· Collect, verify, and enter demographic, financial, and case-related information into the case management system.
· Assess basic eligibility and urgency; provide information about available services and next steps.
· Triage and refer clients to appropriate internal programs or external partners; schedule follow-up appointments.
· Maintain client confidentiality and comply with relevant legal and funder requirements (HIPAA, GDPR as applicable).
· Prepare and submit intake documentation, consent forms, and necessary releases.
· Communicate with probation officers that clients are enrolled in services.
Qualifications
(Required)
· High school diploma or equivalent required; associate’s degree in social services, human services, or related field preferred.
· 1+ year of experience in client intake, customer service, social services, or a related setting preferred.
· Strong verbal and written communication skills.
· Basic computer skills; experience with case management or ERM systems preferred.
· Ability to handle sensitive information and maintain professional boundaries.
